Description The Clinical Program Manager for the PORT Team will provide oversight of day to day outreach activities through supervision of the Peer Outreach Specialists on the PORT Team and collaboration with the Pasadena Public Health Department. The Clinical Program Manager manages all administrative and program reporting, tracks program outcomes and ensures accuracy and timeliness of program documentation. The Clinical Program Manager is also responsible for providing clinical services on the team, including field-based mental health and substance use assessments, short-term and crisis interventions, client care plans, clinical consultations, and resource linkage. | ||||||||||||||||||||

Skills *Must be registered with the BBS and a Licensed Clinical Social Worker in the State of CA. *A minimum of two years’ experience providing outreach or case management support to persons experiencing homelessness, or at risk of homelessness, including working with persons experiencing chronic health issues, mental health needs, or with histories of substance use. *At least one year of supervisory experience in a social service organization is preferred. *Bi-lingual English/Spanish preferred. | ||||||||||||||||||||
