The Brooklyn Transitions Program is a specialized borough-wide prevention program for families challenged by substance use and/or mental health issues. The Social Worker/Case Worker provides home-based, intensive, strength-based, comprehensive services using the Solution-Based Casework (SBC) framework.
Reports To: Social Work Supervisor
Location: 503 5th Avenue, Brooklyn, NY 11215
Hours: 35 hours- Non Exempt (Position requires Staff to be onsite. Hybrid schedules are at discretion)
Salary: $50,000 - $55,000 Annually Based on Relevant Experience******
