Program Description: In Homes Now

In Homes Now is a cluster of scatter site permanent housing programs with 257 apartment units in The Bronx, Manhattan and Brooklyn. The program serves men, women, and families. The individuals and families have histories of homelessness, mental health and substance use. The multidisciplinary team includes case managers, peer counselors, psychiatric care providers, and occupational therapy interns who work with clients to ensure housing stability and improve wellbeing.

Essential Duties and Responsibilities:

Escort clients to medical appointments, benefits appointments, and other appointments as needed
Carry a small caseload and conduct home visits with clients
Assist clients with everyday activities such as grocery shopping and money management
Facilitate groups for clients at the office
Conduct bi-annual housing inspections and view potential new housing units
Maintain accurate and up-to-date clinical documentation and complete case notes in a timely manner
Attend and participate in all staff and team meetings, and in-service training, case consultation with interdisciplinary staff
Assist with front desk lunch coverage (1-2 hours per week) and as needed

Qualifications:

High School Diploma or equivalent and experience with homeless population required;
Strong time management and writing skills;
Spanish Fluency preferred
Valid Driver’s License is a Plus
