As an ECM (Enhanced Care Management) Lead Care Manager, you’ll play a key role in improving health outcomes by coordinating comprehensive community-based services for Medi-Cal beneficiaries. Why This Role Matters:

As an ECM Lead Care Manager, you’ll coordinate access to 14 Community Supports services, directly impacting lives by helping clients navigate healthcare systems and ensuring they receive essential care and support. This role offers personal and professional growth, collaboration, and innovation.

Responsibilities:
Care Coordination: Develop and lead comprehensive care plans, integrating services across providers.
Case Management: Assess needs, develop tailored plans, and manage cases with a focus on medical, social, and support services.
In-Person Visits: Conduct regular visits to assess and support members.
Resource Management: Identify and optimize resources to meet patient needs.
Patient Advocacy: Safeguard patient rights and preferences.
Communication: Act as the central hub for information exchange among all parties.
Documentation: Maintain accurate records, adhering to standards.
Continuous Improvement: Identify areas for enhancement and implement strategies.
Regulatory Compliance: Ensure care delivery meets all legal standards.
Professional Development: Stay ahead of industry trends through ongoing training.

Qualifications:
2+ years in case management or social services
Knowledge of Medi-Cal, CalAIM, ECM, and Community Supports
Strong interpersonal, communication, and organizational skills
Familiarity with healthcare systems and community resources
Cultural competence and sensitivity to diverse populations
