Location: CYC Rebecca Crown Center at 7601 S. Phillips Ave (Chicago, IL)
Schedule: 8:30a - 5:00p; Monday - Friday; schedule varies depending on program needs
Purpose: The Outreach Worker supports resources and referrals to referred DCFS youth and foster families toward the goal of stabilizing placements and assisting youth in the achievement of educational goals; network and interface with assigned POS foster care agency and Caseworker to ensure coordination of educational and developmental programs and services with the child's service plan.
Responsibilities:
Provide and coordinate a continuum of case management services for ages 6 through 19 years old foster care youth; development of effective working relationship with youth and foster parent; completion of all intake and initial assessment protocols; conducting periodic home visits to foster home.
Responsible for the development of collaborative relationships with POS agency staff, school officials, and related organizations and educational resources; coordination of services with all parties; ensuring that appropriate referrals and linkages are made with all resources identified.
Responsible for ensuring that educational assessments and intervention plans are provided to participating youth as needed.
Responsible for the provision of outreach services to foster parents inclusive of counseling, parenting training, and other support services as needed.
Responsible for ensuring that truant youth or dropouts are re-engaged, improving school attendance, developing goals for completion of high school, and for achievement of CPS core content areas.
Responsible for monitoring of youths’ educational progress and participation in agency, POS, CPS or other staffing; acting as an educational advocate within established educational systems inclusive of IEP process, as needed.
