Brief Description of Principal Activities
Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles. St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.


Position Summary
The Case Manager II works as a team Lead member with several social service professional and paraprofessional staff ensuring the delivery of quality care in accordance to the rules and regulations established as well as adhering to the mission of St. Joseph Center.

ESSENTIAL DUTIES & RESPONSIBILITIES

The Case Manager II key areas of responsibility include:

•        Provide street-based outreach, navigation, and supportive services to individuals experiencing homelessness.
•        Develop and maintain strong ties to the community, law enforcement, the Coordinated Entry System (CES), and other homeless service providers.
•        Link clients to appropriate bridge/interim housing programs and assist them in completing required paperwork to enroll them in permanent housing-related programs.
•        Lead Quadrant team members in outreach service areas and special events.
•        Coordinate daily quadrant outreach schedules.

QUALIFICATIONS
•        Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people including clients, community, and other staff. Must have excellent organizational skills and the capability to work in a fast-paced environment.   
•        Strong knowledge of homeless services and resources. Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First Model.
•        Strong leadership and organizational skills and boundary setting
•        A bilingual background is a plus.
•        The position requires an ability to work flexible hours including some early mornings and evenings.
•        Two years’ experience providing street-based services to highly vulnerable populations.
•        Two years’ experience providing supportive services and submitting programs and community based referrals.
•        Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook)
•        Ability to use widely supported internet browsers.
•        Proficient in Clarity/HMIS system navigation.

Education, Certificates, Licenses and Registrations:
•        Bachelor’s degree from an accredited college or university in social work, human services or a related field; a combination of education and experience may be substituted.
•        Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
•        Valid automobile liability insurance.

Notes
Travel Requirements:
•        Travel to meet with clients as needed.
•        Travel between St. Joseph Center businesses to offsite meetings and trainings as needed.
•        Travel to meetings at multiple St. Joseph Center sites and at community partner locations as needed.
