Program Description: The Intensive Mobile Treatment (IMT) Team provides treatment and support services for individuals with frequent interaction with the mental health, substance abuse, criminal justice and homeless services systems and have been poorly served by traditional treatment models. IMT teams stay connected to individuals regardless of their location in shelter, on the street, in housing, hospital or jail. Purpose of Position: The position of Program Director is to provide administrative and clinical oversight of program operations for the IMT Team. The Program Director will practice according to the principles of recovery oriented and trauma informed service delivery.
Schedule: Monday through Friday 9:00 a to 5:00 pm
1. Program Oversight and Quality Assurance
Oversee clinical care and ensuring the highest quality of care
Serve as liaison to NYS Office of Mental Health and NYC DOHMH
Oversee program operations to ensure alignment with regulations and guidelines set forth by the NYS Office of Mental Health (OMH), the NYC Department of Health and Mental Hygiene (DOHMH) and contracted Managed Care Organizations (MCOs)
Oversee new client assessment and assignment
Maintain oversight of utilization review, incident management and monitor outcomes for quality improvement
Develop and maintain relationships with Assisted Outpatient Treatment (AOT), DOHMH Single Point of Access (SPOA), local hospitals, supportive housing, medical clinics and other community providers
Prepare program reports
Develop program budget and monitor program expenses against budget in partnership with fiscal department
Oversee program evaluations and improvement plans
2. Staff Supervision
Meet regularly with staff for clinical supervision; evaluate their work and discuss progress of their clients
Provide guidance on client care issues and agency policies and arrange for training as needed
Arrange staff schedules and ensure adequate coverage, including managing on call schedule
3. Provide case management services
Carry a small caseload.
Perform client assessments and formulate client centered treatment goals
Facilitate client participation in entitlement programs, psychiatric services, medication management, rehabilitation and other activities
Refer clients to community health, substance abuse psychiatric and other services
Accompany clients to appointments as needed
Communicate regularly with clients’ families about their needs and progress
Serve as liaison to other service providers
Maintain up-to-date client case records according to program guidelines
Visit clients in residential facilities, on the street and in other locations
4. Community and Agency Liaison
Serve as liaison to GRCC management and program directors and relate agency-wide information to staff
Serve as liaison to related community programs
5. Other duties as required
Qualifications/Educational Requirements
Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counseling (LMHC) required
SIFI (Seminar In Field Instruction) certification preferred
